Notice of Security Breach
Community Memorial Healthcare (“CMH”) has become aware of a network security incident that may have resulted in the inadvertent exposure of personal and medical information. Although at this time there is no evidence of sensitive information being misused, CMH has taken steps to send letters to anyone whose information may have been exposed.
On October 18, 2023, CMH detected and stopped a network security incident resulting in unauthorized access to individuals’ personal and medical information. Upon learning of the incident, CMH immediately engaged specialized third-party forensic and technical resources to respond to the incident and determine if any information was impacted. CMH reviewed all locations where sensitive data could have been viewed, and then arranged for notification of all individuals whose sensitive data could have been exposed. This investigation concluded December 29, 2023.
While there is no evidence of sensitive information being misused, CMH remains committed to protecting sensitive data. The company has taken steps to send letters to anyone whose information may have been exposed. Letters sent to potentially impacted individuals include additional information about what occurred, credit monitoring services being offered, and a toll-free number where individuals can learn more about the incident. The call center is available Monday through Friday between 8:00 a.m. to 8:00 p.m. EST at 1-844-425-0568.
