CMH offers two Home Care agencies: CMH Certified Home Health Care serves acutely ill patients; CMH LTC Licensed Agency provides long-term management for individuals to remain independently at home.

CMH Certified Home Health Care
Community Memorial Healthcare (CMH) Home Health helps ill or disabled patients recover from an acute illness by offering a broad range of medical care and support services intermittently provided in the patient's home. Home Health services range from simple assistance in activities of daily living to high-tech medical procedures.
Registered nurses (RNs) work with your family physician to provide home health care services which may include: monitoring medications, changing dressings and monitoring patient health; providing instruction for care to patients, family members and friends; and supervising other home health care providers.
Home Health Aides
CMH Certified Home Health Aides can assist with activities such as bathing, dressing and meal preparation. Grooming and attention to personal needs is a vital component of our service. Home Health Aides may also help with basic nursing tasks.
Physical therapists will come to the patient's home to provide services including an evaluation by the therapist to develop a treatment plan to include therapeutic exercise, gait training, balance and coordination exercises, or to train the patient in the use of a walker or cane.
Occupational therapists help patients regain day-to-day skills such as eating, dressing, housekeeping and personal hygiene. The therapist will suggest any adaptive equipment to promote independence.
Speech therapy can help patients improve speaking, swallowing problems or improving cognitive skills following a stroke or head injury.

Community Medical Equipment, part of CMH, can provide all necessary durable medical equipment and supplies to patients.

All of our aides are certified home health aides. We contract through the Northeast Kansas Area Agency on Aging. Please call us for more information.
Who Can Have Home Care?
Services are available to anyone residing in the Northeast Kansas area who has a need for skilled medical services and meets home care guidelines. With a doctor's order, Home Care can provide health care to many patients, from infants to the elderly.
What Will Insurance Cover?
Your insurance company may provide complete or partial payment. The Home Health Care team will explain your coverage and will bill your insurance directly. We accept:
